Privacy Policy

The purpose of this privacy policy is to provide you with a description of the type of information collected by Doug Burfod, LLC (DB LLC), and how we use that information. Your privacy is very important to us. Your privacy and its protection are very important to us. Please read this “Privacy Policy” completely, as well as our ‘Terms and Conditions’ in order to understand how we use information that we may gather. 1. Why we collect information:

We collect information that is necessary or useful in providing you with counseling and/or personal or relationship coaching, including the scheduling, conduct, and billing of related sessions, whether in person or through electronic means, or through written correspondence, resource provision, courses, workshops, retreats, referrals, educational experiences, or other means.

2. What Information we may collect:

Information we may collect includes address(es), telephone number(s), e-mail address(es), emergency contact(s), reason(s) for enlisting our services, condition of mental, physical and spiritual health, family-of-origin information, prescription and other drug or chemical usage, moods, and significant life experiences. Other information may emerge in the course and conduct of sessions, whether through talk therapy, questionnaires, worksheets, or other instruments. That which is, or could be, relevant to your care and growth will be recorded in our confidential, secure, HIPAA compliant electronic storage database.

3. Sharing of Data with Third Party(ies)

Information that you share through any of the above means, including the fact that you are a client, is treated as confidential, with the following exceptions:

Business: Pertinent client information will be utilized for the purposes of conducting business (scheduling, billing, contact, etc.). Information given to us in payment for services, such as checks and credit cards, will be shared with the financial institution(s) and processing centers associated with the transmission of funds from your account(s) to ours. Such transactions and use of personal information will be subject to the Privacy Policies, Terms and Conditions of those entities.

Assessments and therapeutic tools: Third party assessments and therapeutic tools may at times be engaged to diagnose, focus, or otherwise benefit your therapeutic process. In such cases, third party entities may require your first and/or last name, and email address for enrollment. They will then ask you directly for information consistent with the purposes of the assessment, resource, or tool, etc.

Credit Card Companies:

As in any transaction, your credit card company will have all relevant information about the name of the affiliate, products purchased, date and total cost. DB LLC will not otherwise provide any personal data to your credit card company.

Payment Gateways:

Credit card numbers are shared with our partners with whom you place an online order. DB LLC and our payment gateways follow careful procedures to safeguard credit card data in connection with your transactions.

Insurance:

If your insurance company is being billed for services, they will be provided with the minimum information required per your claim.

Consultations: If it is in your interest, SFO may consult with other clinicians or professionals, in order to garner information or resources that may help you achieve your goals. In such cases, your name and private information will be kept confidential; only that which is pertinent to the consultation will be discussed.

Protection: The following situations create a moral or legal obligation by which DB LLC and its practitioners are forced to disclose pertinent information: 1. In cases of suspected abuse or neglect of a minor or of a dependent adult. The appropriate state agency will be notified. 2. If you express intent to harm someone, that person or persons will be warned. Authorities may be notified. 3. If you threaten suicide or disclose someone else’s suicidal ideation, steps will be taken to protect you or someone else from suicide.

Groups: Some sessions are conducted in a group format. If you participate in a group related to DB LLC, then you, DB LLC staff or volunteers, and other group members commit to maintaining the confidentiality of group members’ identities and of information shared, as well as information communicated to other clients. The honoring of confidentiality is the responsibility of group members and cannot be guaranteed by DB LLC.

Judicial and Administrative Proceedings: If you are involved in a legal proceeding and a request for information or records is made, the confidentiality of your information will be protected to the extent allowed by law. Information will be released only with your written permission or that of your legal agent, or in response to a court order, or in response to a legally established program such as Workers’ Compensation, which is endowed with legal authority to make such requests in the determination of benefits. We will disclose information to satisfy government laws and regulations, and to cooperate with legal investigations.

Lawsuits: If a client files a lawsuit against this practice, information may be disclosed in the course of a legal defense.

Electronic Communications: It is impossible to guarantee the protection of confidential information that is stored or transmitted electronically. We will take all reasonable steps to protect the security of electronic information, but can make no guarantee that such information will never be accessed without authorization.

Before DB LLC shares your information with a third party other than those listed above, your verbal or written permission will be sought in-person, by phone, or via email, text, or other electronic means.

4. What use do we make of the information we collect?

In addition to the above, we may use collected information to schedule or change appointments, follow-up on sessions, contact you with information or with questions, subscribe you to the SoulCounsel blog, and notify you of resources or events that may be of interest to you. You may opt out of the blog subscription, promotional notifications, and contact unrelated to your specific case, and may request other means of contact for scheduling purposes and for the conduct of business.

6. How do we use cookies?

You have the ability to accept or decline cookies by modifying your browser settings. Our website is viewable even if you disable the cookie function on your browser. If cookies are enabled on your web browser, then small amounts of data will be stored on your computer when you visit ours or any website. Cookies do not contain any personally identifiable information about you. Cookies track internet usage and can enable websites to customize content according to a visitors’ preferences. At this point in time, DB LLC does not make any use of data collected by cookies.

7. Links

DB LLC may offer links to or from websites it controls or to other websites that it does not control, which provide or review products or information that might be of value to you or other clients. To click on or connect to these links is to come under the Policies and Provisions, Terms of Service, and Refund Policies of such websites.

8. Changes to Privacy Policy

We will occasionally update this “Privacy Policy” as necessary to protect you, and to adjust to and comply with changing realities. We recommend that you check the policy when you visit our site to be sure that you understand the prevailing policy. If we make material changes to this policy we will prominently post such changes prior to implementing the change(s). We will not make material changes in how we may handle previously collected personal information, unless legally required to do so. We will e-mail registered clients concerning material changes to this policy.

9. Consent to Extensions and Exceptions

We will ask for your consent before using information for a purpose other than those set forth in this Privacy Policy.
